MCLA and/or its providers accept many insurance plans. However, due to the constant changes
within the insurance industry, we recommend you check with your insurance company for current provider information. If you need a referral from your primary care physician or mental health network, please make those arrangements.
Please have your insurance card available at the time of your appointment and notify the staff of any changes. We cannot bill your insurance company on your behalf without including the information on your card. If we cannot bill your insurance company, payment is your responsibility at the time of your visit.
Your copayment or deductible amount is due at the time of your appointment.
If you are a self-pay patient, payment is due at the time of service. Please contact the staff before your appointment to determine the amount due.
